Turning Apple Watch into a 12-Lead ECG Recorder

Apple Watch changed how people track their heart health, but it still records only a single-lead ECG. It helps with rhythm detection, but it is not close to what doctors use in a clinic. That gap is exactly where this app comes in.

At Spykra Technologies, we are building an iPhone app that works with your ECG-enabled Apple Watch to capture a full 12-lead ECG. The same kind used in hospitals. The app guides you through placement of the Apple Watch on specific chest locations to record V1 to V6 leads. From those, it automatically calculates limb leads like Lead I, II, III and derived leads like aVR, aVL and aVF using validated formulas.

How it works

The Apple Watch already has high quality electrical sensors. The only limitation is its position. Instead of keeping it on your wrist, the app asks you to place it at different chest locations. Every position gives one ECG segment. The app shows clear instructions for each step.

Typical guided placement:

  • V1: right side of the sternum, fourth rib space
  • V2: left side of the sternum, same level
  • V3: midway between V2 and V4
  • V4: mid clavicular line, fifth rib space
  • V5: horizontal line from V4, anterior axillary line
  • V6: same level, mid axillary line

You hold the watch in each position for a few seconds. The app captures the ECG signal and stores it. Once all leads are collected, the app generates the remaining leads mathematically and builds a complete 12-lead ECG report.

No wires. No gel. No hospital visit. Just your Apple Watch and your iPhone.

What makes it different

This is not another “fitness” ECG graph. A single-lead ECG helps monitor rhythm. A 12-lead ECG helps detect conditions like ventricular hypertrophy, bundle branch blocks, ischemic patterns, ST changes and more. It offers a far deeper picture of heart function.

We are using advanced signal processing and interpretation algorithms to highlight possible patterns such as:

  • Sinus or atrial rhythm issues
  • ST segment elevation or depression
  • Ventricular stresses
  • Conduction delays
  • Ectopic beats
  • Ischemic hints

The readings can be exported as a proper ECG report, with lead labels, calibration grid, and timestamps. Reports can be shared with doctors for professional review.
